| Patients Seen in this Practice |
This practice specializes in evaluation and management of skin
disease related to occupational exposures and to environmental
allergens. This includes Contact Dermatitis.
Extensive Patch Testing (level 4) is available. |

| Requirements to be Seen |
- Referral from a dermatologist, occupational medicine physician,
case manager (for workman's compensation cases) or other healthcare
worker is required to be seen in this clinic.
- If you are referred by a Workman's Compensation Case Worker,
the name of the physician assigned to your case is also required.
You will NOT be seen if the name of the doctor is not provided.
- A letter from the referring physician/healthcare worker is
required.
